Thank you for your interest in UGS 4950 or CSIP.   Follow the steps below to apply for the program.

1. Log into Handshake 
2. Click on Career Center in the top right corner 
3. Click on Experiences 
4. Click on Submit an Experience  
5. Fill out the relevant information including downloading the 3 or 4 forms on the side. These forms are: Student information Sheet, Letter to employer, Student learning agreement, Repeating course agreement (only if you have already done this course before).
6. Email Suzy Shurtliff all completed forms. Email: suzy.shurtliff@utah.edu 

Please Note: If you are international student, you will need to submit a CPT request to ISSS before participating in the internship. Contact ISSS for CPT questions. 

Once you are in the Handshake system and have sent the relevant forms, Suzy can issue you a permission code for registration. You will be able to register for 1-6 credits based on the chart below. The assignments stay the same. Only the hours worked vary.
