WAIVER OF ACCESS INFORMATION
Purpose of the Waiver of Access Form
The Waiver of Access Form ensures that your letters of recommendation are kept confidential as required by most graduate programs and faculty positions. By signing a Waiver of Access Form, you waive your right to view the accompanying letter of recommendation.
What Makes a Letter Confidential?
To be considered confidential, each letter of recommendation submitted to the Credentials Office must be accompanied by a signed Waiver of Access Form. The form can be picked up in person at the Career Services front desk or downloaded below. The Waiver of Access Form is a 2-sided document (2-page attachment when downloaded) that requires the signatures of both parties (you and your recommender).
Checklist
The following is a checklist to ensure that your letters are confidential:
- Have a confidential credential file established with the Credentials Office.
- Obtain one Waiver of Access Form (in person or downloaded) for each letter of recommendation you will be requesting.
- Write your name and student ID number at the top of the form where indicated.
- Sign the bottom of the form on both sides where indicated.
- Present the form to the recommender at the time you make your letter request and indicate where his/her signature is needed. The recommender may either write the letter on the form itself or attach the form to a letter written on his/her professional letterhead.
- Ask the recommender to mail the letter accompanied by the signed Waiver of Access Form directly to:
